COVID - 19 information in different languages:

  • Majority of our appointments are now telephone consultations, please do not show up at the practices without appointments
  • All letters (sicknotes, forms etc) will be posted to you
  • For the interim SARS/ medical requests will be posted to you
  • We are screening patients via phone and at our practice doors as we need to ensure you and our staff are protected
  • Medication - all prescriptions will be sent via EPS. Please ensure you have nominated a pharmacy

We are getting a lot of calls about the NHS England vulnerability/shielding letter to help people with shopping or getting shopping delivery slot. We want to clarify that this letter does not provide you with a code for shopping. For shopping/delivery slots needs please contact Thurrock council

Self Isolation related Sickness Certificates

You do not require a doctor's sickness certificate for any illness lasting seven days or less. Your employer may however require you to complete a self-certification form (SC2) which is available from your employer or on the HMRC website.

If you suspect COVID-19 infection and are having to self-isolate, you may download a digitally signed Sickness Letter to provide to your employer -> Sickness Letter
